Prenatally diagnosed large ovarian cyst in a newborn – A case report

Abstract

Aim: To present the course and outcome of a prenatally diagnosed large simple ovarian cyst in a newborn, with emphasis on the possibility of conservative management without surgical intervention.

Case report: A female neonate from an uneventful pregnancy was delivered at 38 weeks of gestation by elective cesarean section due to transverse lie and polyhydramnios. In the third trimester, a cystic formation measuring 60 mm was detected in the region of the left ovary. At birth, the newborn was in normal general condition with mild abdominal distension, but without respiratory and/or gastrointestinal symptoms. Ultrasound and magnet confirmed a simple anechoic cyst without signs of torsion, hemorrhage, or solid components. Laboratory evaluation, including tumor markers and hormonal assays, was within referral limits. Based on the clinical status and multidisciplinary consultation, conservative management was initiated. Serial ultrasound follow-up showed gradual regression of the cyst, from 50 mm to 20 mm over three months, without complications. No surgical intervention was required.

Conclusions: This case confirms that even large, simple ovarian cysts in neonates may follow a favorable course with conservative treatment. Cyst size alone should not be the sole criterion for surgery. An individualized approach based on clinical status and sonographic features enables preservation of ovarian tissue and avoids unnecessary procedures.
